No entanto, num canto remoto da prov\u00edncia da Zamb\u00e9zia, a sua determina\u00e7\u00e3o revelou-se mais forte do que a corrente que amea\u00e7ava mant\u00ea-la isolada. Esta \u00e9 a hist\u00f3ria da luta inabal\u00e1vel de uma m\u00e3e pela sa\u00fade da sua fam\u00edlia.<\/span><\/p>\n<p><span style=\"color: #333333;\"><\/span><\/p>\n<p><span style=\"color: #333333;\">Rosinha \u00e9 uma mulher solteira de 35 anos do distrito de Namacurra, na prov\u00edncia da Zamb\u00e9zia, que vive na remota ilha de Marroda, na localidade de Mbaua. \u00c9 m\u00e3e de quatro filhos, dois dos quais, Milagre (13) e T\u00e9rcia (11), tamb\u00e9m vivem com o HIV. Desde muito cedo, Rosinha enfrentou a dura realidade do isolamento geogr\u00e1fico e da informa\u00e7\u00e3o limitada sobre sa\u00fade. A unidade de sa\u00fade mais pr\u00f3xima ficava do outro lado de um rio que tinha de ser atravessado em canoas improvisadas. Esta viagem perigosa e<\/span><\/p>\n<p><span style=\"color: #333333;\"><\/span><\/p>\n<p><span style=\"color: #333333;\">As chuvas frequentes no seu distrito faziam com que o caudal do rio aumentasse, tornando a travessia ainda mais dif\u00edcil, e a falta de vias de acesso alternativas manteve-a afastada da unidade sanit\u00e1ria. Sem cuidados pr\u00e9-natais adequados, Milagre nasceu com HIV. Dois anos depois, a hist\u00f3ria se repetiu. Durante a sua segunda gravidez, Rosinha evitou os servi\u00e7os de preven\u00e7\u00e3o da transmiss\u00e3o vertical do HIV. Apesar do aconselhamento dos profissionais de sa\u00fade, ela continuou a faltar \u00e0s consultas, o que resultou em longos per\u00edodos sem medica\u00e7\u00e3o. Consequentemente, a sua segunda filha, T\u00e9rcia, tamb\u00e9m foi diagnosticada com HIV.<\/span><\/p>\n<p><span style=\"color: #333333;\"><\/span><\/p>\n<p><span style=\"color: #333333;\">Com o tempo, a sa\u00fade de Milagre foi-se deteriorando. Ele sofria de diarreia constante, crescimento lento e falta de apetite - sinais de alerta que Rosinha j\u00e1 n\u00e3o podia ignorar. Desesperada, recorreu \u00e0 medicina tradicional, mas os resultados foram escassos; Milagre continuava a adoecer.<em>\u201cJ\u00e1 n\u00e3o conseguia dormir. Sentia-me culpada por n\u00e3o ter seguido os conselhos da unidade sanit\u00e1ria\u201d, recorda Rosinha. O peso da sua culpa era enorme, mas a mudan\u00e7a parecia ainda fora de alcance, at\u00e9 ao dia em que atravessou o rio e conheceu uma equipa de agentes comunit\u00e1rios de sa\u00fade.<\/em>O peso da sua culpa era enorme, mas a mudan\u00e7a parecia ainda fora de alcance, at\u00e9 ao dia em que atravessou o rio e conheceu uma equipa de agentes comunit\u00e1rios de sa\u00fade.<\/span><\/p>\n<p><span style=\"color: #333333;\"><\/span><\/p>\n<p><span style=\"color: #333333;\">Os agentes de sa\u00fade trabalhavam na organiza\u00e7\u00e3o de um evento de mobiliza\u00e7\u00e3o da comunidade, onde eram dadas palestras sobre preven\u00e7\u00e3o e tratamento do HIV. Motivada, Rosinha decidiu abordar a um deles, informando-o sobre a doen\u00e7a do seu filho. O agente comunit\u00e1rio de sa\u00fade ofereceu-se para  acompanh\u00e1-la a casa e  ver de perto o estado da crian\u00e7a. \u201cEsse gesto mudou tudo para mim. Percebi que n\u00e3o estava sozinha e que podia lutar pela vida dos meus filhos\u201d, diz Rosinha. Ao ver o estado da crian\u00e7a, o agente de sa\u00fade prometeu apoi\u00e1-la para que ela e o filho pudessem visitar a unidade sanit\u00e1ria no dia seguinte. Sem hesitar, Rosinha aceitou a ajuda e dirigiu-se \u00e0 unidade sanit\u00e1ria de Mbaua, onde era esperada pela equipe de provedores de sa\u00fade.<\/span><\/p>\n<p><span style=\"color: #333333;\"><\/span><\/p>\n<p><span style=\"color: #333333;\">Rosinha e o seu filho foram recebidos na unidade sanit\u00e1ria e, devido \u00e0 sua fraca ades\u00e3o ao tratamento, foram inscritos no aconselhamento de ades\u00e3o refor\u00e7ada durante tr\u00eas meses. Esta estrat\u00e9gia centra-se nos pacientes em TARV que faltam frequentemente \u00e0s consultas, interrompem o tratamento ou t\u00eam carga viral n\u00e3o controlada. Durante a primeira sess\u00e3o, uma equipa de profissionais de sa\u00fade - incluindo um psic\u00f3logo, um cl\u00ednico e um educador de pares avaliou as raz\u00f5es subjacentes \u00e0s interrup\u00e7\u00f5es recorrentes do tratamento de Rosinha. Os principais obst\u00e1culos identificados foram as barreiras de transporte, a dist\u00e2ncia e as dificuldades psicossociais, que conduziram a um plano personalizado para reintegrar Rosinha nos cuidados de sa\u00fade para o HIV.<\/span><\/p>\n<p><span style=\"color: #333333;\"><\/span><\/p>\n<p><span style=\"color: #333333;\">Milagre e T\u00e9rcia foram submetidos a uma avalia\u00e7\u00e3o cl\u00ednica para detectar doen\u00e7as relacionadas com o HIV. Nesse mesmo dia, as crian\u00e7as foram integradas nos servi\u00e7os de tratamento antirretroviral (TARV). Rosinha recebeu medica\u00e7\u00e3o TARV para os tr\u00eas, bem como um plano de apoio nutricional para os seus filhos. Para garantir o acesso cont\u00ednuo da sua fam\u00edlia ao TARV, foi designada uma m\u00e3e mentora para dar apoio comunit\u00e1rio e evitar mais interrup\u00e7\u00f5es no tratamento. Numa das viagens para a unidade de sa\u00fade, a canoa que transportava Rosinha, Milagre e T\u00e9rcia virou no rio. As tr\u00eas foram resgatadas pela comunidade, mas o trauma permanece. Mesmo assim, Rosinha nunca desistiu. \u201cO medo foi imenso, mas o meu amor pelos meus filhos \u00e9 mais forte. N\u00e3o posso desistir\u201d, diz com firmeza.<\/span><\/p>\n<p><span style=\"color: #333333;\"><\/span><\/p>\n<p><span style=\"color: #333333;\">Os profissionais de sa\u00fade continuaram a monitorizar Rosinha e os seus dois filhos regularmente, assegurando que seguiam o plano de tratamento. Com o apoio da sua fam\u00edlia e da sua m\u00e3e mentora, Rosinha conseguiu um acesso consistente \u00e0 medica\u00e7\u00e3o. Com o passar do tempo, a sa\u00fade de Milagre e T\u00e9rcia estabilizou-se, permitindo que Rosinha reduzisse as suas visitas \u00e0 unidade sanit\u00e1ria. Rosinha come\u00e7ou a receber medicamentos para si e para os seus filhos de tr\u00eas em tr\u00eas meses, o que ajudou bastante pela localidade remota de onde provinha.<\/span><\/p>\n<p><span style=\"color: #333333;\"><\/span><\/p>\n<p><span style=\"color: #333333;\">Gra\u00e7as \u00e0 melhoria na sua sa\u00fade, Rosinha e os seus filhos come\u00e7aram a participar em brigadas m\u00f3veis organizadas na sua comunidade, uma solu\u00e7\u00e3o eficaz para pacientes que vivem em \u00e1reas remotas. Determinada a mudar o seu futuro, Rosinha manteve-se firme no seu tratamento e teve mais dois filhos, ambos nascidos sem HIV.<\/span><\/p>\n<p><span style=\"color: #333333;\"><\/span><\/p>\n<p><span style=\"color: #333333;\">A mobiliza\u00e7\u00e3o comunit\u00e1ria e as brigadas m\u00f3veis promovidas pelas autoridades distritais de sa\u00fade na prov\u00edncia da Zamb\u00e9zia, em parceria com a C-Sa\u00fade e financiadas pelo PEPFAR, foram fundamentais para transformar a vida de Rosinha. \u201cQuando os profissionais de sa\u00fade v\u00eam at\u00e9 n\u00f3s, sentimos que as nossas vidas s\u00e3o valorizadas. \u00c9 um al\u00edvio enorme para muitas fam\u00edlias\u201d, explica.<\/span><\/p>\n<p><span style=\"color: #333333;\"><\/span><\/p>\n<p><span style=\"color: #333333;\">Hoje, Rosinha Eug\u00e9nio \u00e9 mais do que uma simples utente dos servi\u00e7os de sa\u00fade. \u00c9 uma l\u00edder na sua comunidade, uma inspira\u00e7\u00e3o para outras mulheres e a prova viva de que o amor e a resili\u00eancia podem ultrapassar at\u00e9 as maiores adversidades. Rosinha is a 35-year-old single woman from Namacurra district in Zamb\u00e9zia province who lives on the remote island of Marroda, in the locality of Mbaua. She is the mother of four children, two of whom, Milagre (13) and T\u00e9rcia (11), are also living with HIV. From an early age, Rosinha faced the harsh reality of geographical isolation and limited health information. The nearest health facility was located on the other side of a river that had to be crossed in improvised canoes. This perilous and exhausting journey proved a constant obstacle for anyone seeking medical care. Growing up without guidance on disease prevention and treatment, Rosinha saw her vulnerability turn into a concrete challenge in 2011 when, pregnant with her first child, she was diagnosed with HIV. The impact of the diagnosis was overwhelming. Unable to accept her condition, Rosinha stopped her antiretroviral treatment while continuing her pregnancy follow-up at the maternal and child health services. Frequent rains in her district caused the river\u2019s flow to increase, making crossing even more difficult, and the lack of alternative access routes kept her away from the health facility. Without adequate prenatal care, Milagre was born with HIV. Two years later, history repeated itself. During her second pregnancy, Rosinha avoided HIV mother-to-child transmission prevention services. Despite counseling from health professionals, she continued missing clinical appointments, resulting in long periods without medication. Consequently, her second child, T\u00e9rcia, was also diagnosed with HIV. Over time, Milagre\u2019s health deteriorated. He suffered from constant diarrhea, slow growth, and poor appetite\u2014warning signs that Rosinha could no longer ignore. In desperation, she turned to traditional medicine, but it yielded few results; Milagre continued to fall ill. \u201cI couldn\u2019t sleep anymore. I felt guilty for not following the health facility\u2019s advice,\u201d Rosinha recalls. The weight of her guilt was enormous but change still seemed out of reach\u2014until the day she crossed the river and met a team of community health workers. These workers were organizing a community mobilization event, where talks on HIV prevention and treatment were given. Motivated, Rosinha decided to approach one of them, informing him about her child\u2019s illness. The community health worker offered to visit her at home and accompany her to see the child. \u201cThat gesture changed everything for me. I realized I wasn\u2019t alone and that I could fight for my children\u2019s lives,\u201d says Rosinha. Seeing the child\u2019s condition, the worker promised to support her so that she and her child could visit the health facility the next day. Without hesitation, Rosinha accepted the help and went to Mbaua health facility, where the worker was waiting for her. Rosinha and her child were received at the health facility and, because of their poor adherence to treatment, were enrolled in Enhanced Adherence Counseling (EAC) for three months. This strategy focuses on patients on ART who frequently miss appointments, interrupt treatment, or have uncontrolled viral loads. During the first EAC session, a team of health professionals including a psychologist, a clinician, and a peer educator\u2014assessed the underlying reasons for Rosinha\u2019s recurrent treatment interruptions. The main obstacles identified were transport barriers, distance, and psychosocial difficulties, which led to a personalized plan to reintegrate Rosinha into HIV care. Milagre and T\u00e9rcia, underwent a clinical evaluation, checking for HIV-related illnesses. That same day, the children were integrated into antiretroviral therapy (ART) services. Rosinha received ART medication for the three of them, as well as a nutritional support plan for her children. To ensure her family\u2019s continued access to ART, a mentor mother was assigned to provide community support and prevent further treatment interruptions. For the first time, Rosinha felt empowered to face her condition and ensure that her children received the care they needed. However, the road to recovery was not without obstacles. On one of their trips to the health facility, the canoe carrying Rosinha, Milagre, and T\u00e9rcia capsized in the river. The three were rescued by the community, but the trauma lingers. Still, Rosinha never gave up. \u201cThe fear was immense, but my love for my children is stronger. I can\u2019t give up,\u201d she says firmly. Health professionals continued to monitor Rosinha and her two children regularly, ensuring they followed the treatment plan. Supported by her family and her mentor mother, Rosinha managed consistent access to medication. Over time, Milagre and T\u00e9rcia\u2019s health stabilized, allowing Rosinha to reduce her health facility visits. She began receiving medicines for herself and her children every three months, which greatly helped life in such a remote location. Thanks to the improvements in their health, Rosinha and her children began participating in mobile brigades organized in her community\u2014an effective solution for clients living in remote areas. Determined to change her future, Rosinha remained steadfast in her treatment and had two more children, both born HIV-free. Community mobilization and mobile brigades promoted by the district health authorities in Zamb\u00e9zia province, in partnership with C-Sa\u00fade and funded by PEPFAR, were instrumental in transforming Rosinha\u2019s life. \u201cWhen health providers come to us, we feel our lives are valued. It\u2019s a huge relief for many families,\u201d she explains. Today, Rosinha Eug\u00e9nio is more than just a health service user. She is a leader in her community, an inspiration to other women, and living proof that love and resilience can overcome even the greatest adversities. In desperation, she turned to traditional medicine, but it yielded few results; Milagre continued to fall ill. \u201cI couldn\u2019t sleep anymore. I felt guilty for not following the health facility\u2019s advice,\u201d Rosinha recalls. The weight of her guilt was enormous but change still seemed out of reach\u2014until the day she crossed the river and met a team of community health workers. These workers were organizing a community mobilization event, where talks on HIV prevention and treatment were given. Motivated, Rosinha decided to approach one of them, informing him about her child\u2019s illness. The community health worker offered to visit her at home and accompany her to see the child. \u201cThat gesture changed everything for me. I realized I wasn\u2019t alone and that I could fight for my children\u2019s lives,\u201d says Rosinha. Seeing the child\u2019s condition, the worker promised to support her so that she and her child could visit the health facility the next day. Without hesitation, Rosinha accepted the help and went to Mbaua health facility, where the worker was waiting for her. Rosinha and her child were received at the health facility and, because of their poor adherence to treatment, were enrolled in Enhanced Adherence Counseling (EAC) for three months. This strategy focuses on patients on ART who frequently miss appointments, interrupt treatment, or have uncontrolled viral loads. During the first EAC session, a team of health professionals including a psychologist, a clinician, and a peer educator\u2014assessed the underlying reasons for Rosinha\u2019s recurrent treatment interruptions. The main obstacles identified were transport barriers, distance, and psychosocial difficulties, which led to a personalized plan to reintegrate Rosinha into HIV care. Milagre and T\u00e9rcia, underwent a clinical evaluation, checking for HIV-related illnesses. That same day, the children were integrated into antiretroviral therapy (ART) services. Rosinha received ART medication for the three of them, as well as a nutritional support plan for her children. To ensure her family\u2019s continued access to ART, a mentor mother was assigned to provide community support and prevent further treatment interruptions. For the first time, Rosinha felt empowered to face her condition and ensure that her children received the care they needed. However, the road to recovery was not without obstacles. On one of their trips to the health facility, the canoe carrying Rosinha, Milagre, and T\u00e9rcia capsized in the river. The three were rescued by the community, but the trauma lingers. Still, Rosinha never gave up. \u201cThe fear was immense, but my love for my children is stronger. I can\u2019t give up,\u201d she says firmly. Health professionals continued to monitor Rosinha and her two children regularly, ensuring they followed the treatment plan. Supported by her family and her mentor mother, Rosinha managed consistent access to medication. Over time, Milagre and T\u00e9rcia\u2019s health stabilized, allowing Rosinha to reduce her health facility visits. She began receiving medicines for herself and her children every three months, which greatly helped life in such a remote location. Thanks to the improvements in their health, Rosinha and her children began participating in mobile brigades organized in her community\u2014an effective solution for clients living in remote areas. Determined to change her future, Rosinha remained steadfast in her treatment and had two more children, both born HIV-free. Community mobilization and mobile brigades promoted by the district health authorities in Zamb\u00e9zia province, in partnership with C-Sa\u00fade and funded by PEPFAR, were instrumental in transforming Rosinha\u2019s life. \u201cWhen health providers come to us, we feel our lives are valued. It\u2019s a huge relief for many families,\u201d she explains. Today, Rosinha Eug\u00e9nio is more than just a health service user.
