O desenvolvimento e implementa\u00e7\u00e3o do sistema foi apoiado pelo Plano de Emerg\u00eancia do Presidente dos EUA para o Al\u00edvio da SIDA (PEPFAR) atrav\u00e9s dos Centros de Controlo e Preven\u00e7\u00e3o de Doen\u00e7as dos Estados Unidos (CDC). O iDMED est\u00e1 actualmente em uso em 356 unidades sanit\u00e1rias em Mo\u00e7ambique.<\/span><\/p>\n<p><span style=\"color: #333333;\"><\/span><\/p>\n<p><span style=\"color: #333333;\">Durante anos, as unidades sanit\u00e1rias apoiadas pelo PEPFAR em Mo\u00e7ambique utilizavam um sistema eletr\u00f3nico de farm\u00e1cia baseado num computador, utilizado principalmente para a distribui\u00e7\u00e3o de medicamentos anti-retrovirais (ARV). O sistema anterior, apesar de ser uma linha de vida para os antiretrovirais, dependia de instala\u00e7\u00f5es locais e de tecnologia desactualizada, o que dificultava a expans\u00e3o e a interoperabilidade.<\/span><\/p>\n<p><span style=\"color: #333333;\"><\/span><\/p>\n<p><span style=\"color: #333333;\">Reconhecendo estas defici\u00eancias, o Minist\u00e9rio da Sa\u00fade, o PEPFAR e a C-Sa\u00fade uniram for\u00e7as para conceber o iDMED, o Sistema Inteligente de Dispensa de Medicamentos. A nova plataforma foi constru\u00edda de raiz como uma solu\u00e7\u00e3o segura, baseada na Web, capaz de gerir todos os medicamentos essenciais, e n\u00e3o apenas os ARV. Utilizando frameworks modernas como o Vue.js no front end, Spring Boot no back end, APIs RESTful para uma comunica\u00e7\u00e3o perfeita e OpenHIM para interoperabilidade nacional, os desenvolvedores da C-Sa\u00fade criaram um sistema que suporta opera\u00e7\u00f5es online e offline, garantindo que mesmo os postos de sa\u00fade mais isolados nunca percam o acesso a informa\u00e7\u00f5es e funcionalidades cr\u00edticas.<\/span><\/p>\n<p><span style=\"color: #333333;\"><\/span><\/p>\n<p><span style=\"color: #333333;\">A implementa\u00e7\u00e3o seguiu uma abordagem deliberadamente faseada, come\u00e7ando em unidades sanit\u00e1rias de menor volume, onde a migra\u00e7\u00e3o de dados e a forma\u00e7\u00e3o do pessoal puderam ser supervisionadas de perto. Durante a fase piloto, as equipas cl\u00ednicas e de farm\u00e1cia forneceram retro informa\u00e7\u00e3o em tempo real sobre a usabilidade, permitindo que a equipa t\u00e9cnica aperfei\u00e7oasse os fluxos de trabalho antes de expandir a plataforma para unidades sanit\u00e1rias mais movimentadas. Essa estrat\u00e9gia cuidadosa, centrada nas pessoas, garantiu que os servi\u00e7os nunca fossem interrompidos; Os profissionais de sa\u00fade fizeram a transi\u00e7\u00e3o de forma tranquila, com treinamentos pr\u00e1ticos e a confian\u00e7a de que o cuidado ao paciente melhoraria, ao inv\u00e9s de ser prejudicado.<\/span><\/p>\n<p><span style=\"color: #333333;\"><\/span><\/p>\n<p><span style=\"color: #333333;\">Os resultados t\u00eam sido verdadeiramente transformadores. O registro em tempo real e a gest\u00e3o automatizada de stocks foram aprimorados, enquanto os registros digitais evitam erros de transcri\u00e7\u00e3o, elevando tanto a tomada de decis\u00e3o cl\u00ednica quanto a seguran\u00e7a do paciente. Sempre que ocorre uma queda na rede, as unidades simplesmente mudam para o modo offline, mantendo toda a funcionalidade at\u00e9 que a conex\u00e3o seja restabelecida. E, como as actualiza\u00e7\u00f5es acontecem automaticamente atrav\u00e9s da interface web, a manuten\u00e7\u00e3o do sistema melhorou significativamente, liberando recursos de TI para outras tarefas essenciais. Talvez o mais importante seja que a integra\u00e7\u00e3o do iDMED com o sistema eletr\u00f4nico de acompanhamento dos pacientes (baseado no OpenMRS) oferece uma vis\u00e3o abrangente da jornada de cada paciente, facilitando as refer\u00eancias e apoiando decis\u00f5es pol\u00edticas baseadas em dados em todos os n\u00edveis do sistema de sa\u00fade.<\/span><br \/><span style=\"color: #333333;\">\u201c<em>Podemos acompanhar os movimentos de stock em tempo real e responder instantaneamente a situa\u00e7\u00f5es de escassez. Whenever a network outage occurs, clinics simply switch to offline mode, maintaining full functionality until their connection returns. And because updates occur automatically through the web interface, system maintenance improved dramatically freeing IT resources for other vital tasks. Rafique Eduardo is one of the hundreds of users of iDMED, which is the pharmacy information system developed by Centro pela Sa\u00fade Global (C-Sa\u00fade) in partnership with the Ministry of Health. The development and roll-out of the system has been supported by the U.S. President\u2019s Emergency Plan for AIDS Relief (PEPFAR) through the United States Centers for Disease Control and Prevention (CDC). iDMED is currently in use at 356 health facilities throughout Mozambique. For years, the PEPFAR supported health facilities in Mozambique had been using a desktop-based electronic pharmacy system primarily used for antiretroviral (ARV) drug dispensing. The previous system while a lifeline for antiretrovirals relied on local installations, and outdated technology, hindering expansion and interoperability. Recognizing these weaknesses, the Ministry of Health, PEPFAR and C-Sa\u00fade joined forces to design iDMED, the Intelligent Medication Dispensing System. The new platform was built from the ground up as a secure, web-based solution capable of managing every essential medication, not just ARVs. By leveraging modern frameworks Vue.js on the front end, Spring Boot on the back end, RESTful APIs for seamless communication, and OpenHIM for national interoperability C-Sa\u00fade developers created a system that supports both online and offline operations, ensuring that even the most isolated health posts never lose access to critical information and functionalities. Rollout followed a deliberately phased approach, beginning in lower-volume clinics where data migration and staff training could be closely supervised. During each pilot phase, pharmacy and clinical teams provided real-time feedback on usability, allowing the technical team to refine workflows before extending the platform to busier facilities. This careful, people-centered strategy meant services never skipped a beat; health workers transitioned smoothly, armed with hands-on training and the confidence that patient care would improve rather than be interrupted. The results have been nothing short of transformative. Real-time registration and automated stock management improved, while digital records guard against transcription errors, elevating both clinical decision-making and patient safety. Whenever a network outage occurs, clinics simply switch to offline mode, maintaining full functionality until their connection returns. And because updates occur automatically through the web interface, system maintenance improved dramatically freeing IT resources for other vital tasks. Perhaps most importantly, iDMED\u2019s integration with electronic patient tracking system (based in OpenMRS) offers a comprehensive view of each patient\u2019s journey, smoothing referrals and underpinning data-driven policy decisions at every level of the health system.\u201cWe can track stock movements in real time and respond instantly to shortages. Even in our busiest centers, where the system sometimes slows briefly, the overall efficiency gains have revolutionized how we deliver care,\u201d explains D\u00e1rio Osmel Victor Gabriel, an ICAP-supported user in Nampula province. \u201cStock management has dramatically improved, facilitating the production of reports required by the Ministry of Health\u201d adds K\u00e1tia Zacarias Chissano a pharmacy technician in Namacata health facility in Zambezia province while she is preparing her monthly inventory report. PEPFAR support in Namacata health facility has transitioned to the provincial health authorities of Zamb\u00e9zia and iDMED is essential in the provision of accurate reports. C-Sa\u00fade is a Mozambican association and serves as the central partner of PEPFAR for Health Information Systems (HIS) in Mozambique.
