Durante muitos anos, o sistema de sa\u00fade de Mo\u00e7ambique enfrentou grandes desafios na gest\u00e3o e disponibiliza\u00e7\u00e3o atempada de resultados laboratoriais essenciais, como a carga viral, fundamentais para o seguimento cl\u00ednico das pessoas que vivem com HIV. O processo era lento e manual: os resultados gerados no Sistema de Informa\u00e7\u00e3o Laboratorial (DISA) precisavam ser impressos e entregues aos cl\u00ednicos, ou inseridos manualmente, um a um, no Sistema Eletr\u00f3nico de Seguimento de Pacientes (SESP). Erros de digita\u00e7\u00e3o, falta de informa\u00e7\u00e3o e longos tempos de espera faziam parte da rotina di\u00e1ria.<\/span><\/p>

Filomena Vili recorda os dias em que precisava regressar v\u00e1rias vezes \u00e0 unidade sanit\u00e1ria apenas para obter o resultado da sua carga viral. \u201cPor vezes voltava tr\u00eas vezes e ainda n\u00e3o havia resultado\u201d, conta. Hoje, tudo mudou. A liga\u00e7\u00e3o autom\u00e1tica entre o DISA e o SESP permite agora que os resultados laboratoriais sejam transmitidos eletr\u00f3nica e instantaneamente para o prontu\u00e1rio de cada paciente, gerando o e-Lab, um formul\u00e1rio laboratorial eletr\u00f3nico.<\/span><\/p>

Atualmente, assim que o exame \u00e9 conclu\u00eddo no laborat\u00f3rio de refer\u00eancia, geralmente a centenas de quil\u00f3metros da unidade sanit\u00e1ria, os cl\u00ednicos podem aceder aos resultados em tempo real.<\/span><\/p>

Mais do que um avan\u00e7o tecnol\u00f3gico, esta integra\u00e7\u00e3o representa um passo estrat\u00e9gico rumo ao alcance das metas 95-95-95: garantir que 95% das pessoas que vivem com HIV conhe\u00e7am o seu estado serol\u00f3gico, 95% estejam em tratamento e 95% alcancem a supress\u00e3o viral.<\/span><\/p>

Inicialmente lan\u00e7ada na prov\u00edncia da Zamb\u00e9zia, com 144 unidades sanit\u00e1rias sincronizadas, a plataforma de interoperabilidade expandiu-se e atualmente abrange todas as prov\u00edncias do pa\u00eds, com mais de 500 unidades sanit\u00e1rias a utilizar o sistema e mais de um milh\u00e3o de resultados de carga viral transmitidos ao SESP nos \u00faltimos 12 meses. The Clinical Summary app is a secure, mobile application directly linked to the health facility\u00b4s Electronic Patient Tracking System, allowing clinicians to access patient information including viral load results during clinical consultations. For many years, Mozambique\u2019s health system faced major challenges in managing and providing timely access to essential laboratory results, such as viral load, that are crucial for the clinical follow-up of people living with HIV. The process was slow and manual: results generated in the Laboratory Information System (DISA) had to be printed and handed over to the clinicians or entered one by one into the Electronic Patient Tracking System (SESP). Typing errors, missing information, and long waiting times were part of the daily routine. Filomena Vili recalls the days when she had to return to the health facility multiple times just to get her viral load result. \u201cSometimes I would come back three times, and still there was no result,\u201d she says. Today, things have changed. As soon as her sample is collected, it is entered in the DISA system and once processed at a referral laboratory, the clinical officer at her facility can immediately see the result directly on his phone. To transform this reality, the Ministry of Health of Mozambique, with support from the U.S. Government and CDC, in partnership with C-Sa\u00fade Health Information Systems team and other implementing partners, invested in innovative digital interoperability solutions. The automatic connection between DISA and SESP now allows laboratory results to be transmitted electronically and instantly to each patient\u2019s medical record, generating the e-Lab, an electronic laboratory form. Today, once a test is completed at the referral laboratory, usually hundreds of miles away from the health facility, clinicians can access the results in real time. \u201cBefore, we used to wait for days; now, on the same day, I can counsel the patient and make clinical decisions based on up-to-date information,\u201d explains L\u00facio Assis, a Clinical Officer at 24 de Julho Health Center. The impact is visible: waiting times from sample collection to receiving the results have been reduced from weeks to days, strengthening the quality of care provided. Data quality and integrity have also improved thanks to the employment of applications that use global standards, such as HL7, which prevent duplication and enable the correction of identified discrepancies. The integration with SESP also makes it possible to track each result and ensure that no information is lost. In Quelimane, L\u00facio adds: \u201cInteroperability has completely changed our work. Today, we treat people with more confidence\u2014we know the system is working with us, not against us.\u201d More than just a technological advancement, this integration represents a strategic step toward achieving the 95-95-95 targets: ensuring that 95% of people living with HIV know their status, 95% are on treatment, and 95% achieve viral suppression. Initially launched in Zamb\u00e9zia Province with 144 synchronized health facilities, the interoperability platform has now expanded to cover all provinces across the country with more than 500 health facilities implementing the system, and a million plus viral load results transmitted to SESP in the past 12 months. C-Sa\u00fade\u2019s interoperability team has been instrumental in the design and development of the solution, along with technical support to clinical implementing partners throughout the country to implement the solution. The DISA\u2013SESP interoperability stands as a symbol of collaboration and vision\u2014a collective achievement of the Ministry of Health, C-Sa\u00fade, the U.S. Government, dedicated health professionals, and every person on antiretroviral treatment, who now receives their results at the right time. .
