<\/p>\n

No passado, durante \u00e9pocas chuvosas como a que estamos a viver agora, Ruquina Cunfersso passava dias, semanas ou at\u00e9 meses sem conseguir levantar os medicamentos que lhe permitem viver uma vida saud\u00e1vel e ativa e cuidar dos seus dois filhos. Hoje, a dispensa semestral da terapia antirretroviral (TARV) em 100 unidades sanit\u00e1rias da Zamb\u00e9zia transformou o acesso ao tratamento para pacientes est\u00e1veis, como Ruquina, sobretudo quando as estradas ficam inundadas e as fam\u00edlias est\u00e3o mais ocupadas com o trabalho agr\u00edcola.<\/p>\n

<\/p>\n

Ruquina soube que vivia com HIV em 2009, depois de procurar cuidados de sa\u00fade por se sentir mal. O diagn\u00f3stico foi dif\u00edcil de aceitar \u2014 e tornou-se ainda mais doloroso quando o seu falecido marido recusou iniciar o tratamento. Ruquina manteve-se em seguimento porque queria permanecer saud\u00e1vel para cuidar dos seus filhos \u2014 e continuar a trabalhar para os sustentar. Durante a \u00e9poca chuvosa, por\u00e9m, que coincide com o per\u00edodo de trabalho mais intensivo na machamba, era muito dif\u00edcil chegar \u00e0 unidade sanit\u00e1ria mais pr\u00f3xima, o Centro de Sa\u00fade de Macuse, situado a cerca de 20 quil\u00f3metros da sua parcela. Como resultado, sofreu longas interrup\u00e7\u00f5es no tratamento, o que provocou flutua\u00e7\u00f5es na sua carga viral e o enfraquecimento da sua sa\u00fade geral.<\/p>\n

Agricultora de 40 anos e vendedora de \u00e1gua gelada e fritos, Ruquina aderiu ao modelo de dispensa semestral de TARV em outubro de 2025 e j\u00e1 n\u00e3o se preocupa com o facto de as vias de acesso \u00e0 unidade sanit\u00e1ria poderem ficar cortadas pelas chuvas, que na Zamb\u00e9zia normalmente duram de outubro a mar\u00e7o de cada ano.<\/p>\n

<\/p>\n

A dispensa semestral de TARV reduz o tempo gasto em desloca\u00e7\u00f5es e em filas de espera (e, consequentemente, o tempo produtivo perdido), bem como os custos de transporte. Os benef\u00edcios da dispensa de medicamentos antirretrovirais duas vezes por ano (em vez de mensal ou trimestralmente) estendem-se tamb\u00e9m aos profissionais de sa\u00fade e \u00e0s unidades sanit\u00e1rias, refletindo-se numa gest\u00e3o mais eficaz do tempo e de outros recursos (incluindo o stock de farm\u00e1cia), em cuidados mais centrados nas pessoas e na reten\u00e7\u00e3o em TARV, contribuindo, em \u00faltima inst\u00e2ncia, para a redu\u00e7\u00e3o da depend\u00eancia do sistema nacional de sa\u00fade de financiamento externo.<\/p>\n

Crimilde Macuelia, t\u00e9cnica cl\u00ednica, explica que, com a introdu\u00e7\u00e3o da dispensa semestral de TARV, \u201cforam observadas mudan\u00e7as muito importantes no funcionamento do Centro de Sa\u00fade de Macuse\u201d. Entre as mudan\u00e7as mais significativas, Crimilde destaca uma redu\u00e7\u00e3o consider\u00e1vel das longas filas de espera, a melhoria do seguimento dos pacientes e o aumento do n\u00famero de pessoas retidas nos cuidados e tratamento do HIV. \u201cQuando as filas diminuem, o aconselhamento melhora\u201d, acrescenta, \u201ce o seguimento torna-se mais direcionado\u201d.<\/p>\n

Enquanto benefici\u00e1ria destes servi\u00e7os, Ruquina confirma: \u201cA dispensa semestral de TARV trouxe muitas melhorias para a minha sa\u00fade e muito mais tranquilidade. Agora consigo organizar a minha vida familiar, o trabalho na machamba e o meu neg\u00f3cio sem a press\u00e3o de ter de ir \u00e0 unidade sanit\u00e1ria todos os meses\u201d, afirma. Com acesso est\u00e1vel \u00e0 medica\u00e7\u00e3o, Ruquina sente-se agora forte, saud\u00e1vel e \u201cmuito bonita\u201d.\nRuquina \u00e9 uma das mais de 127.000 pessoas que vivem com HIV, estavam ativas em TARV e inscritas na dispensa semestral de TARV numa das 100 unidades sanit\u00e1rias da Zamb\u00e9zia que oferecem este modelo de presta\u00e7\u00e3o de servi\u00e7os. Today, six-month antiretroviral therapy (ART) dispensation at 100 health facilities in Zamb\u00e9zia has transformed treatment access for stable patients, like Ruquina, especially when roads flood and families are busiest with farm work. Ruquina learned that she was living with HIV in 2009, after seeking care because she felt unwell. The diagnosis was difficult to accept\u2014made even harder when her late husband refused treatment. She stayed in care because she wanted to remain healthy for her children\u2014and to keep working to provide for them. During the rainy season, however, which coincided with the most intensive period of work on her field, it was very difficult to get to the nearest health facility of Macuse, located about 20 kilometers away from her plot. Today, six-month antiretroviral therapy (ART) dispensation at 100 health facilities in Zamb\u00e9zia has transformed treatment access for stable patients, like Ruquina, especially when roads flood and families are busiest with farm work. Ruquina learned that she was living with HIV in 2009, after seeking care because she felt unwell. The diagnosis was difficult to accept\u2014made even harder when her late husband refused treatment. She stayed in care because she wanted to remain healthy for her children\u2014and to keep working to provide for them. During the rainy season, however, which coincided with the most intensive period of work on her field, it was very difficult to get to the nearest health facility of Macuse, located about 20 kilometers away from her plot. Photographer: Solina Ribeiro A 40-years old farmer and seller of chilled water and fried snacks, Ruquina enrolled in the six-month ART dispensation model in October 2025 and no longer worries that access roads to the health facility will be cut off by the rains, which in Zamb\u00e9zia typically last from October to March each year. Six-month ART distribution reduces the time spent traveling and waiting in queues (and therefore productive time lost), in addition to transportation costs. The benefits of dispensing antiretroviral medicines twice a year (rather than monthly or quarterly) extend to providers and health facilities as well, reflected in more effective management of time and other resources (including pharmacy stock), more human-centered care, and retention on ART, ultimately reducing the national health system\u2019s dependence on external funding. Crimilde Macuelia, a Clinical Officer, explains that with the introduction of six-month ART distribution, \u201cvery important changes have been observed in the functioning of Macuse Health Center.\u201d Among the most impactful changes, Crimilde highlights a significant reduction in long queues, improved patient follow-up, and an increase in the number of patients retained in HIV care and treatment. \u201cWhen queues are reduced, counselling improves,\u201d she adds, \u201cand follow-up becomes more targeted.\u201d As a recipient of these services, Ruquina confirms: \u201cSix-month ART dispensation has brought many improvements to my health and much greater peace of mind. I can now organize my family life, my farm work, and my business without the pressure of having to go to the health facility every month,\u201d she says. With stable access to medication, Ruquina now feels strong, healthy, and \u201cvery beautiful\u201d.Ruquina is one of more than 127,000 people living with HIV who were active on ART and enrolled in six-month ART distribution at one of the 100 health facilities across Zamb\u00e9zia that offer this delivery model.
