Em Moçambique, onde a tuberculose continua a ser um desafio crítico de saúde pública entre as crianças, a história de Eva Evaristo, uma menina de 2 anos de Namacurra, na província da Zambézia, demonstra a implementação eficaz de abordagens inovadoras de diagnóstico e tratamento apoiadas pelo Programa Nacional de Controlo da Tuberculose, financiado pelo PEPFAR.
A jornada de Eva começou quando a sua mãe, a Sra. Goi Luís, percebeu que Eva tinha uma febre persistente e estava a emagrecer. Uma visita inicial à unidade sanitária de Naciaia na sua localidade, não revelou resultados conclusivos, levando a Sra. Goi a procurar uma avaliação mais aprofundada na unidade sanitária de Namacurra sede. Considerando o historial familiar de Eva: A sua avó estava em tratamento de tuberculose a equipa de saúde levantou a hipótese de que ela também pudesse estar infectada pela doença.
Confirmation came through an unconventional test: a stool sample. Diagnosing tuberculosis in children is particularly challenging due to the difficulty they face in providing sputum samples. In response, the National Tuberculosis Control Program, with support from C-Saúde and funding from PEPFAR, introduced an alternative diagnostic approach using stool samples. Despite initial reservations, Mrs. Goi understood the importance of the test and, with the help of the health team, was able to collect the sample. The result, days later, confirmed what everyone feared: Eva had tuberculosis.
A Sra. Goi recorda: “Foi um choque saber que a minha filha tinha a mesma doença da minha mãe. Mas, ao mesmo tempo, senti um alívio por saber que havia cura.” Após o diagnóstico, Eva iniciou com o tratamento da tuberculose.
Mrs. Goi observed early changes in her daughter’s condition, explaining, “At first, I received medication for a month, and I soon noticed improvements. My daughter was more energetic, her eyes began to shine again, and her appetite returned.” Alongside medication, Eva’s treatment plan included nutritional and mental health support. These integrated services were crucial given the challenges Mrs. Goi faced in managing the long-distance travel to health facilities and the ongoing risk of reinfection within the household.
Refletindo sobre as dificuldades vividas neste período, a Sra. Goi afirmou: “Foi difícil, mas como mãe, nada me impediu de fazer o melhor pela minha filha. Eu sabia que a cura estava ao nosso alcance e tinha que lutar por ela.”
After several months of consistent treatment and follow-up, Eva’s condition improved markedly. Her recovery was evidenced by her renewed energy and a return to normal activities, attesting to the success of both the medication regimen and the novel diagnostic strategy. Mrs. Goi expressed her relief and gratitude: “Now, my daughter is cured and healthy. She is a girl full of joy. As a mother, I feel immense happiness seeing her well again. “Eva’s illustrates the tangible benefits of adopting stool-based diagnostic methods for pediatric tuberculosis. Between January and December 2024, the implementation of stool diagnosis resulted in a significant increase in the number of confirmed diagnoses: from 8 TB identified cases in 126 samples collected among children in the first quarter of fiscal year 2025 to 54 cases identified in 485 samples in the fourth quarter. This change not only saved Eva’s life but also influenced her grandmother, who found motivation in her granddaughter’s recovery to continue her own treatment.
“É fundamental que as comunidades partilhem experiências como a nossa, para que as pessoas saibam que a tuberculose tem cura.” afirma a Sra. Goi, enfatizando a importância da sensibilização. Agora, o próximo passo é garantir que a Eva não seja reinfectada, e a Sra. Goi está comprometida em seguir todas as orientações de prevenção.
A história de Eva é um farol de esperança, um exemplo do impacto positivo que o diagnóstico precoce e o tratamento adequado podem ter na vida de uma criança. A estratégia de diagnóstico da tuberculose por fezes é apoiada pela C-Saúde com fundos do PEPFAR, actualmente tem se tornado numa ferramenta crucial na luta contra a tuberculose.

"O tratamento da Eva foi um sucesso. Olhando para trás, percebo como esta experiência me tornou mais forte. O mais importante foi seguir todas as orientações dos provedores de saúde. Minha filha agora está bem de saúde, e sou grata por tudo o que aprendi neste processo de cuidado e tratamento," Sra. Goi
